If drugstore cleansers and prescription creams have not settled stubborn breakouts, you already know acne is more than a surface problem. Topical retinoids, benzoyl peroxide, and other guideline-directed medicines can treat acne well for many people. They are not a failed first step. They are still the foundation of acne care.

When inflammatory lesions keep returning, or when oiliness and post-acne texture linger after a solid topical plan, an in-office protocol can sit alongside that care. At Premier Med Spa of Lake Oswego, that protocol is not a generic “acne laser.” It is a multi-modality program built around ClearLift, often with microdermabrasion and a curated SkinCeuticals home-care regimen.

What Premier actually uses for acne

Laser acne treatment here centers on ClearLift, a non-ablative Q-switched Nd:YAG laser on our Alma platform. The principal wavelength in this family is 1064 nm. We say that so the mechanism stays honest: this is not a 1726 nm sebaceous-gland laser, and it is not a blue-light or photodynamic treatment built for a different chromophore.

ClearLift energy is delivered beneath the surface while the outer layer stays intact. On our acne visits, that energy is directed toward the sebaceous glands to help calm oil production and to gently resurface post-acne texture. A complementary laser may be paired to address surface debris and support collagen renewal. We do not invent a second brand name for that handpiece. Candidacy for pairing is decided at consultation.

Microdermabrasion is often added first. It exfoliates the surface and opens pores so the laser portion of the visit can reach the skin we actually intend to treat. New acne patients receive a complimentary SkinCeuticals LHA cleanser, toner, and serum to continue a consistent routine at home.

That combination is the differentiator. The visit is not one mystery wavelength doing every job.

How 1064 nm evidence fits (and where it does not)

Acne is multifactorial: sebum, follicular keratinization, C. acnes, inflammation, hormones, and genetics all play a role. A laser does not switch that system off. It may help reduce some of the factors driving active inflammatory acne, and individual results vary.

Published 1064 nm Nd:YAG studies have found reductions in inflammatory lesions and sebum with this wavelength family. Those papers include Q-switched protocols and a sham-controlled 650 microsecond 1064 nm trial. They support talking about 1064 nm for acne. They do not set Premier’s pulse width, fluence, or visit schedule, and we will not copy another clinic’s protocol onto ClearLift.

We also will not import another device’s mechanism and describe it as ClearLift. If a paper is about a different wavelength, it does not belong in this explanation.

Topicals still matter

It is tempting to tell a sales story in which creams stay on the surface and lasers reach the “real” problem. That is misleading.

Guideline-directed topical therapy can reach the follicle at therapeutic concentrations for many patients. Oral medicines remain appropriate for some presentations. Procedural care is increasingly discussed as a complement to that medical therapy, not a replacement for it. If a topical plan is working, we will not talk you out of it. If it is stalling, or if texture and oiliness remain, ClearLift acne care is one option to evaluate in person.

What a visit feels like

Care starts with a consultation. We look at your breakout pattern, what you have already used, and whether laser therapy is a fit. A small number of people do not respond to laser acne treatment. That is why we are honest up front instead of promising a series that works for everyone.

Each visit begins with a thorough cleanse, then microdermabrasion when it is indicated, then the laser portion. Sessions are well tolerated. You can return to normal activity afterward. That is minimal downtime, not a promise that the skin will look untouched. Mild redness can happen. Sun protection still matters.

Acne care is delivered as a planned series rather than a single visit. Many patients enjoy long remissions. Others do better with routine maintenance. We map the exact plan at consultation. We do not publish a one-size session count or interval.

After active breakouts: texture and scars

Calming active inflammation is one job. Softening the look of leftover texture is another. Once active lesions are quieter, we can talk about laser scar reduction with Morpheus8 and ClearLift. No device erases scars completely. Appearance may lighten and smooth over a series of sessions. Individual results vary.
