Premier Med Spa of Lake Oswego ("Premier Med Spa," "we," "us," or "our") respects your privacy and is committed to being transparent about how we collect, use, and share information. This Privacy Policy ("Policy") explains our information practices in connection with your use of https://go2medspa.com/ and any related web pages, forms, and online services we operate that link to this Policy (collectively, the "Site").

Please read this Policy carefully. By accessing or using the Site, submitting information through the Site, or otherwise interacting with us online, you acknowledge that you have read and understood this Policy. If you do not agree with our practices, please do not use the Site.

This Policy works alongside our other notices. Our Terms of Use, Cookie Policy, and Notice of Privacy Practices each govern specific aspects of your relationship with us, and additional terms may apply to particular services. Where a separate notice addresses a topic in greater detail, that notice controls for that topic.

A note about health information. This Policy describes how we handle information collected through our public website and online forms. To the extent any information you provide constitutes protected health information governed by the Health Insurance Portability and Accountability Act ("HIPAA") or other applicable health-privacy laws, our handling of that information is described in our separate Notice of Privacy Practices. Our public website is a general marketing and informational resource and is not intended for the transmission of detailed medical history, diagnoses, or treatment records. Please do not submit sensitive medical details through web forms, email, text message, or chat.

1. Scope of This Policy

This Policy applies only to information collected through the Site and through your direct electronic communications with us (such as web-form submissions, emails, text messages, and calls placed through links on the Site). It does not apply to:

  • Information collected offline or in person at our physical location;
  • Information governed by a separate notice that we provide to you, including our Notice of Privacy Practices;
  • Third-party websites, platforms, or services that may link to or from the Site, each of which maintains its own privacy practices for which we are not responsible.

2. Information We Collect

We collect information in three ways: information you provide to us directly, information collected automatically when you use the Site, and information we may receive from third parties.

2.1 Information You Provide to Us

When you contact us, request an appointment or consultation, sign up for communications, or otherwise interact with our forms and features, you may provide:

  • Contact and identity details — such as your name, email address, telephone number, and any address information you choose to share;
  • Appointment and inquiry details — such as the service or treatment you are interested in, your preferred dates or times, and any message, comments, or questions you submit;
  • Communications — the content of messages you send to us through forms, email, text, or chat, and our responses; and
  • Any other information you voluntarily choose to provide through the Site.

You control the information you share with us through the Site. If you do not wish to provide certain information, you may decline, although this may limit our ability to respond to your inquiry or schedule your visit.

2.2 Information Collected Automatically

When you visit the Site, we and our service providers may automatically collect certain technical and usage information through cookies and similar technologies (see Section 3), including:

  • Device and connection data — such as your IP address, browser type, operating system, device identifiers, and general (city- or region-level) location inferred from your IP address;
  • Usage data — such as the pages you view, the links you click, the date and time of your visit, the pages that referred you to us, and how you navigate and interact with the Site; and
  • Analytics data — aggregated and individual usage metrics generated by analytics tools, including Google Analytics.

2.3 Information From Third Parties

We may receive information about you from third parties, such as analytics and advertising providers, marketing partners, or platforms through which you interact with our content. We may combine this information with information we already hold about you.

3. Cookies, Analytics, and Tracking Technologies

The Site uses cookies, pixels, tags, software development kits, and similar technologies ("Tracking Technologies") to operate the Site, understand how it is used, and support our marketing.

  • We currently use Google Analytics to measure and analyze Site traffic and usage.
  • We may now or in the future use additional Tracking Technologies, including Google Tag Manager, Google Ads conversion and remarketing tags, and the Meta (Facebook) Pixel, to measure the effectiveness of, and deliver, advertising.

These technologies may collect information such as your device and usage data described in Section 2.2 and may share certain information with the providers of those technologies for analytics and advertising purposes. Some of this activity may be considered "targeted advertising", a "sale", or "sharing" of personal data under applicable state privacy laws.

A detailed description of the specific Tracking Technologies we use, their purposes, and how to manage them is provided in our separate Cookie Policy. You can also control many Tracking Technologies through your browser settings and through the opt-out and rights mechanisms described in Sections 6 and 7, including by transmitting a recognized universal opt-out signal such as Global Privacy Control (GPC).

4. How We Use Your Information

We use the information we collect for the following purposes:

  • To respond to your inquiries, requests, and communications;
  • To schedule, confirm, and manage appointments and consultations;
  • To provide, operate, maintain, secure, and improve the Site;
  • To send you information, promotions, offers, and updates you have requested or that relate to our services, where permitted by law;
  • To understand how visitors use the Site and to measure and improve our content, services, and marketing;
  • To personalize your experience and to deliver and measure advertising, including on third-party platforms;
  • To detect, prevent, investigate, and address fraud, security incidents, misuse of the Site, and other unlawful or harmful activity;
  • To comply with applicable laws, regulations, legal processes, and our legal obligations, and to establish, exercise, or defend legal claims; and
  • For any other purpose disclosed to you at the time of collection or to which you consent.

We will not process your information for a materially different purpose without providing notice or, where required, obtaining your consent.

5. How We Share Your Information

We do not sell your personal information for money. We may, however, share information in the following circumstances:

  • Service providers and processors. We share information with vendors who perform services on our behalf — such as website hosting, form processing, communications, scheduling, analytics, marketing, and IT support — and who are permitted to use the information only as needed to provide those services to us.
  • Analytics and advertising partners.We share certain device and usage information with analytics and advertising providers (such as Google and Meta) through the Tracking Technologies described in Section 3. Depending on the configuration of these technologies and applicable law, this may constitute a "sale," "sharing," or "targeted advertising."
  • Legal and protective disclosures. We may disclose information when we believe in good faith that disclosure is necessary to comply with applicable law, regulation, subpoena, court order, or other legal process; to respond to lawful requests from public authorities; to enforce our agreements and policies; or to protect the rights, property, safety, or security of Premier Med Spa, our patients, our personnel, or others.
  • Business transfers. In connection with, or during negotiations of, any merger, acquisition, financing, reorganization, sale of assets, or similar transaction, or in the event of insolvency or bankruptcy, information may be transferred as a business asset, subject to commercially reasonable confidentiality arrangements.
  • With your direction or consent. We may share information for any other purpose with your consent or at your direction.

We may also share aggregated or de-identified information that cannot reasonably be used to identify you.

6. Advertising, Analytics, and Your Choices

We want you to have meaningful choices about Tracking Technologies and advertising:

  • Browser controls. Most browsers allow you to refuse or delete cookies and to clear stored data. Disabling certain cookies may affect how the Site functions.
  • Analytics opt-out. You can opt out of Google Analytics by installing the Google Analytics Opt-out Browser Add-on, available from Google.
  • Advertising opt-outs. You can manage interest-based advertising preferences through industry tools such as the Digital Advertising Alliance (optout.aboutads.info) and the Network Advertising Initiative (optout.networkadvertising.org), and through the ad-settings controls offered by Google and Meta.
  • Universal opt-out signals. We honor recognized universal opt-out mechanisms, including Global Privacy Control (GPC). When we detect a valid GPC signal from your browser or device, we will treat it as a request to opt out of the "sale" of personal data and processing of personal data for "targeted advertising" to the extent associated with that browser or device.

7. Your Privacy Rights

Depending on where you live, you may have certain rights regarding your personal data. We honor the rights described below in accordance with applicable law.

7.1 Oregon Residents (Oregon Consumer Privacy Act)

If you are an Oregon resident acting in an individual or household context, you may have the right to:

  • Confirm whether we process your personal data and access that data;
  • Obtain a list of the specific third parties (or, at our option as permitted by law, categories of third parties) to which we have disclosed your personal data;
  • Correct inaccuracies in your personal data;
  • Delete personal data we hold about you;
  • Obtain a portable copy of personal data you previously provided, in a usable format; and
  • Opt out of the processing of your personal data for purposes of (a) the sale of personal data, (b) targeted advertising, and (c) profiling in furtherance of decisions that produce legal or similarly significant effects.

We process sensitive data — which can include data revealing health information and precise geolocation — only with your consent or as otherwise permitted by law.

7.2 Residents of Other U.S. States

If you reside in another state with a comprehensive consumer privacy law (such as California, Colorado, Connecticut, Texas, Virginia, or others), you may have similar rights, including rights to access, correct, delete, obtain a copy of, and limit certain processing of your personal data, and to opt out of "sales," "sharing," and targeted advertising. We will honor verifiable requests to the extent required by the law applicable to you.

7.3 How to Exercise Your Rights

To submit a request, please contact us using the methods in Section 12. We will take reasonable steps to verify your identity before responding, which may require you to provide information that matches what we already hold. You may use an authorized agent to submit a request on your behalf where permitted by law, subject to proof of authorization.

We will respond to verifiable requests within the timeframe required by applicable law (generally within 45 days, with one additional 45-day extension where reasonably necessary and permitted). We will not discriminate or retaliate against you for exercising your rights.

7.4 Right to Appeal

If we decline to take action on your request, we will explain why. Where required by applicable law, you may appeal that decision by contacting us using the information in Section 12. If we deny your appeal, you may have the right to contact your state Attorney General. Oregon residents may contact the Oregon Department of Justice.

8. Do Not Track and Global Privacy Control

Some browsers transmit "Do Not Track" signals. Because there is no common industry or legal standard for interpreting Do Not Track signals, the Site does not currently respond to them. We do, however, honor recognized universal opt-out mechanisms such as Global Privacy Control (GPC) as described in Section 6.

9. Data Retention

We retain personal data for as long as reasonably necessary to fulfill the purposes described in this Policy, including to respond to your inquiries, provide and improve our services, maintain business records, and comply with our legal obligations, resolve disputes, and enforce our agreements. When personal data is no longer needed, we take reasonable steps to delete, de-identify, or securely store it.

10. Data Security

We maintain reasonable administrative, technical, and physical safeguards designed to protect the information we collect. However, no website, electronic transmission, or method of storage is completely secure, and we cannot and do not guarantee the absolute security of your information. You transmit information to us at your own risk, and you are responsible for keeping any credentials you use confidential. If you have reason to believe your interaction with us is no longer secure, please notify us immediately using the information in Section 12.

11. Children's Privacy

The Site is intended for a general adult audience and is not directed to children. We do not knowingly collect personal data from children under 16 through the Site, and we do not knowingly sell the personal data of, or use for targeted advertising, any consumer we know to be under 16. If you believe a child has provided us with personal data through the Site, please contact us using the information in Section 12 and we will take appropriate steps to delete it.

13. Visitors Outside Oregon and the United States

We operate the Site from the United States. If you access the Site from outside Oregon or the United States, you understand that your information may be processed in jurisdictions that may have data-protection laws different from those in your jurisdiction, and you consent to that processing. The Site is intended for users located in the United States.

14. Changes to This Policy

We may update this Policy from time to time to reflect changes in our practices, technologies, or legal requirements. When we make changes, we will revise the "Last Updated" date above and post the updated Policy on this page. Material changes may be communicated through additional notice where required by law. Your continued use of the Site after an updated Policy becomes effective constitutes your acknowledgment of the changes.
