Jawline Botox and the Platysma Lift in Lake Oswego

When most people think about neuromodulator treatments, forehead lines and crow’s feet usually come to mind first. Yet some of the most useful aesthetic conversations happen further down the face. If you have started noticing a softer jawline, early jowling, or vertical cords along your neck when you speak, the cause may not be loose skin alone. Often it is an active sheet of muscle called the platysma.

Botox treatments in Lake Oswego can be used to reduce platysma activity along the jawline and neck. Clinicians sometimes call this a platysma lift or Nefertiti lift. It is a non-surgical option for vertical neck bands and the way they can blur the jawline. It is not a substitute for surgery when skin laxity or fat is the main problem.

How the platysma changes the jawline

Facial muscles work as opposing groups. Elevators in the midface help lift tissue when you smile. Depressors pull down.

The platysma is the largest depressor in the lower face and neck. It is a thin sheet that starts over the upper chest and collarbones, crosses the neck, and inserts along the lower border of the jaw and nearby skin. Repeated grimacing, speaking, and strain can keep that downward pull active. Over time, the border between jaw and neck can look less sharp.

Why vertical bands show up

As skin loses collagen and elastin, fat shifts, and the platysma can hold a higher resting tone. The central edges of the muscle may separate and thicken into vertical cords: platysmal bands.

When those bands contract, they can pull skin away from deeper neck structure and tether along the mandibular border. That can look like early sagging even when there is little extra fat or loose skin. This article is about those vertical bands, not horizontal etched lines sometimes called tech neck.

What a labeled platysma treatment actually targets

BOTOX Cosmetic (onabotulinumtoxinA) is FDA-approved for the temporary improvement of moderate to severe platysma bands in adults, in addition to its older cosmetic indications for glabellar lines, crow’s feet, and forehead lines. The October 2024 prescribing information describes injections into the upper platysma below the jawline and into the vertical platysma bands.

That is the pattern this article is about. A provider may ask you to grimace so the active cords can be seen and treated. As band activity decreases over the following days, the neck can look smoother when you talk or animate. As platysma activity decreases, downward tension along the lower face is reduced, which can make the mandibular border appear cleaner and more defined. Individual results vary.

DAO is a separate decision

In some patients, a provider may separately evaluate other lower-face depressor muscles, such as the depressor anguli oris, when downturned mouth corners contribute to the overall appearance. That treatment is anatomically distinct from treatment of the platysma. Premier lists DAO Botox as its own option on the Botox page. It is not part of the labeled platysma-band protocol.

Botox vs. Dysport for the neck

Patients often ask whether Dysport is interchangeable with Botox for the neck. Both are prescription botulinum toxin type A products. They are not unit-for-unit equivalent, and labeled uses are not the same.

BOTOX Cosmetic is currently FDA-approved for the temporary improvement of moderate to severe platysma bands in adults. Dysport’s U.S. cosmetic indication is the temporary improvement of moderate to severe glabellar lines in adults younger than 65. Dysport is not FDA-approved for platysma bands. Other botulinum toxin products, including Dysport, may sometimes be used by clinicians in the lower face or neck based on individual anatomy, but those uses may be off-label. Product selection, dosing, and injection patterns are not interchangeable.

Do not assume one product travels farther because of how it is manufactured. Dose, concentration, volume, and technique matter more than a simple spread story. Your provider chooses a product from our injectables options based on anatomy and goals, not a marketing comparison.

What to expect at a Lake Oswego consultation

The lower face and neck include muscles used for chewing, swallowing, and speech. Planning should be unhurried.

Your provider will look at neck movement, skin quality, and how hard the platysma works at rest and in motion. You may be asked to clench, smile, and grimace. We also look for submental fullness or laxity that an injectable cannot fix.

The injection visit is typically completed in minutes. The full office visit is generally 10 to 20 minutes. Effects may begin within 24 to 48 hours, with the full effect visible within two weeks. Results typically last three to four months on average. Most people return to their day after the visit. We usually advise avoiding pressure on treated areas, strenuous activity, and heat for about 24 hours. Individual results vary.

Who is (and is not) a candidate

Neuromodulators affect muscle. They do not remove significant loose skin or a meaningful pad of submental fat. They are not a substitute for surgery when structural laxity is the main finding.

You may be a reasonable candidate if you have visible vertical platysma bands when you speak or grimace, mild to moderate jawline softness from downward muscle pull, and decent skin elasticity.

A platysma lift is less likely to help if you have a large amount of loose skin, substantial fat under the chin, a neuromuscular condition, or if you are pregnant or nursing.

If mild laxity sits next to active bands, we may talk about pairing neuromodulators with a skin-remodeling option after an in-person look. That is a separate plan, not a bundled promise.
